MOH 362 is a standardized form used by health facilities in Kenya to register for HIV testing services. The form collects information on the name of the facility, the location of the facility, and the contact information for the facility. The form also collects information on the type of HIV testing services offered by the facility, such as rapid testing, laboratory testing, and pre-exposure prophylaxis (PrEP).

The MOH 362 form is an important tool for monitoring the availability of HIV testing services in Kenya. The data collected from the form is used to identify areas where there are gaps in access to HIV testing services and to ensure that all Kenyans have access to these services.

Today we shall learn how to generate MOH 362 from Kenya EMR easily with outs struggle.

To Generate this report, makes sure you are logged in to Kenya EMR. Also make sure that you have either refreshed tables or recreated them. this is to help data that you have been working on has been updated with relevant tables. to start off, go to Home then click on Reports

Proceed to common tab; click on HTS Register it will load a page click on Request Report 

after period selection press request

The report generation will start and will be on queue and takes few seconds or minutes depending on your server load and workload.

After a while your report will be ready to use you can download as excel or csv file as shown below
